Program Feedback Forms

The Program Feedback Form (PFF) requires schools and Out of School Hour Care Services (sites) to provide feedback on various aspects of the AASC program and account for funds spent across the whole funding period.

PFFs will be distributed to sites by Active After-school Communities (AASC) zone administration officers at the end of funding periods.

Responsibility for the management and administration of financial records and acquitting of funds lies solely with the site. Please refer to the Program feedback form completion guide for assistance with this process. A completed PFF is to be returned to the respective administration officer within 30 days from the end of the funding period.
