Grievance, injury and incident reporting

Grievance process

Where a significant grievance or concern arises that relates to the Active After-school Communities (AASC) program, school and Out of School Hours Care Services (site) coordinators and community coaches are encouraged to contact their local AASC Regional Coordinator who will assist with resolving the issue.

If the site coordinator or community coach has any concerns regarding the AASC program that have not been resolved to their satisfaction, or if they wish to lodge formal notification of an issue, they can do so by using the Australian Sports Commission's Customer Feedback and Complaints process.  

ASC incident and injury report form

Where an incident or injury relates to an AASC employee or to visitors, contractors, volunteers etc under the duty of care of the ASC, or the injury or incident occurs in an event conducted by the AASC program, AASC staff should use the ASC incident and injury report form and send it to Human Resources (HR). Regional coordinators are required to ensure this form is used for all injuries, incidents and near misses. This particularly relates to:

  • CCTP training conducted by ASC employees
  • Events conducted and coordinated by the ASC
  • Any incident, near miss or accident that occurs within a AASC office or place of work
  • Work related vehicle/transport accidents.

Of particular importance to note is the following:

  • HR must be notified immediately if there is a death of any person
  • HR must be notified immediately if any person sustains an injury which requires the attention of a medical practitioner, visit to casualty or admission to hospital
  • HR must be notified immediately if an ambulance is called to any of the AASC sites.

If an ASC incident and injury report form  has been lodged with HR stating no medical attention was sought at the time of the incident, and in the next few days medical treatment is required in relation to the injury sustained in the incident, HR must be notified as soon as possible.  

Non - ASC incident injury report forms

Where an injury or incident occurs at a site or at an event not conducted by the AASC program, where other organisations hold the duty of care, (example site run programs and activities), the non-ASC Incident report form and Injury report form could be used or organisation specific processes followed. The forms are examples for sites to use. Any completed forms should be kept on file.

If the concern relates to:

  • unlawful harassment or discrimination - a complaint may be lodged at any time or advice sought from the Anti-discrimination Commission
  • suspected child abuse - mandatory reporting may apply. Please refer to National Child Protection Clearinghouse for advice on reporting procedures.
